If you or a loved one are planning life after high school graduation this website was created for you! 

The transition process can feel very complicated and overwhelming for many. If you ever find yourself feeling confused, know that you are not alone. This website was created with the hopes of making planning for transition a little easier for you.

On this website, you will find:

  • A list of local resources for those with special needs in Blair County with contact information and website links. Resources are organized by the type of services they provide. 

  • A flow chart to try to help you understand the process to receiving services. Contact information for some local county Administrative Entities (AEs) are included at the top of the flow chart. AEs serve as the first place you should check with when beginning your search for services.

  • An acronym list with some commonly used acronyms.

  • A timeline with some additional resources as well as general transition timeframe with some transition tips

  • A survey to gain a better understanding of how the website could be improved and the types of additional resources and information that could be helpful to include.

 

Please be aware that while the plan is to update this information yearly, changes may occur. So, always check with your school, Administrative Entity, Supports Coordination Organization or the individual organizations first. This website does not contain a complete list of the resources and processes that may exist but may help provide you with a good idea of where to start.
